My suggestion is not to use שליט"א  (shelita) or (זצ"ל)  zatsal in name
headings (1xx/4xx) because it is not really part of the name and will
obviously change if shelita!  In the 670 where there is a date of
publication present it could be useful (i.e. that the person was still
alive in the year work was published).

in contrast, ha-Levi/ha-Kohen are parts of a name that don't change

Take a look at how Bar Ilan formed the name
