Climate and Diversity Committee survey

Attached is a word document with the questions that will be listed on the upcoming qualtrics survey from the Staff Subcommittee (Jay Gupta, Shelley Palmer, Ruth Leonard) of the Climate and Diversity Committee.  We are sending you the questions first so that you  have some time to think about them prior to completing the survey.  Climate and Diversity has surveyed each of the different constituencies in the department this year and would appreciate your participation when the survey is sent to you.
