[Natfinwellness] Peer Program Materials - Intake form?

Happy winter wishes to all!

For those with established an peer mentor/counseling program, *what intake
documents do you use*? I am looking for any documentation that helps the
peer counselor get to know the student better or outlines overall goals and
expectations.
