Dear Supervisors and Staff,
Reminder: During the months of April and May supervisors are asked to meet one on one with their staff and complete an annual performance review, to discuss the past year’s performance and set goals for the new performance period. Performance Review documents are due on Friday, May 17, 2019.
Performance Review Process:
1. Employee - complete your Self Appraisal form electronically and email to your supervisor in advance to prepare your review. This is the first attachment.
2. Supervisors - complete your staff reviews electronically (type/prepare them)
Note: If you are having difficulty opening either document.
Copy or drag the file to your desktop
Open document and make modifications.
Save utilizing the file name nomenclature indicated in #6
If you are still having difficulties, you may have an older version of the software need and need an update to open and edit without issue. Please contact Technical Support.
1. Employee and Supervisor meet and review performance review, position description and set goals for next year. While this process may seem focused on documentation, the meetings and conversations between supervisors and staff are still the most important component.
1. Supervisor should combine all review documents into one document in the following order:
* the unsigned, electronic 2018/2019 Performance Review
* the 2019/2020 Plan
* the employee’s Self Appraisal document
1. Send combined document to ASC-physics-hr at osu.edu<mailto:ASC-physics-hr at osu.edu>
2. Please use the following file naming scheme: ORG#PEP_name.#
* ORG number (everyone should use 06840)
* PEP (stands for Performance evaluation and plan)
* underscore symbol _
* name.#
Examples:
Pam Hood’s document: 06840PEP_Hood.252
Jean Ball’s document: 06840PEP_ball.1780
Note: Everyone’s will start the same: 06840PEP_
1. Signatures will be collected in DocuSign to all needed parties sent to OSU email addresses. Please note: DocuSign will not forward links to gmail accounts. You will need to sign in through DocuSign using your OSU email address. You can log in directly to DocuSign at https://account.docusign.com/#/username
1. Once the documents are completed everyone who signed will receive an electronic copy in their DocuSign account
Reminders
* Ratings must be selected for each category of the evaluation and an overall rating must be selected.
* Ratings are to be supported by comments in the “Comments” section for any rating category that is below or exceeds expectations but are encouraged for all ratings.
* Please include self-evaluations or employee comments to submit in the same file with the employee’s performance evaluation and plan.
* Use the position description to set expectations and goals
Electronic Performance Review documents are due to the Physics HR office no later than Friday, May 17, 2019. Please email ASC Physics HR<mailto:ASC%20Physics%20HR%20%3cASC-physics-hr at osu.edu%3e> with any questions about the Performance Review process.
