My Tax Form PIN Reset:
With 2015 behind us, tax season is looming! The OSU Payroll/Tax office is aware of issues with the PIN reset process and vendor if you are trying to access www.mytaxform.com<http://www.mytaxform.com>. If you need to log-in and  cannot remember your PIN please call OSU Payroll or OHR directly. The system will tell you to contact your Department Human Resources Professional (HRP); however I do not have access to reset your PIN. You will most likely be asked to provide your 8 or 9 digit OSU Employee ID, and confirm your date of birth or address to proceed with the PIN reset.

Enrollment Closing Soon for Spring Term Dependent Tuition Assistance: January 15, 2016
The Ohio State University offers eligible employees and their dependents the opportunity to advance their education at Ohio State. For dependent tuition assistance, applications must be submitted each term. Spring 2016 term enrollment window closes Friday, January 15. Faculty and staff can apply online using Employee Self Service<http://eprofile.osu.edu/> or paper applications.
