Registration open online for Physics 9/16 Biometric health screenings in 1136 PRB (for PHA)

Faculty and Staff (on OSU Health Plans),

On Sept. 16th,  the OSU Health Plan's Health Promotion Specialist(s) will be in our department, in room 1136 from 8:00-2:30 to do onsite biometric health screenings.

Registration is now open online for Sept. 16 in the Physics Research Building. Click on "Start Here".  Then click "Schedule a biometric screening".  Log in with name.#.  Select "Appointments and Classes" tab on top.  Highlight "Biometric Health Screening".  In the box that states "Appointment Code" please  type  in   PRB9162015   Do NOT copy and paste as this doesn't always work to activate the code.  Then click "Find Appointments or Classes".   This will show only the appointments for your event in your building.  Continue to schedule as usual.  If you experience any difficulty, please contact Customer Service at 292-4700, but you must have your Appointment Code or they will not be able to assist you.

Remember, if you have recently* had a physical or completed a biometric screening with your provider, you can use those values to complete your Personal Health and Well-Being Assessment (PHA). Completing your PHA, with all of the required values (height, weight, blood pressure, total cholesterol, HDL and a blood sugar value (either glucose or A1C)), by September 30, 2015 will allow you to receive 2016 medical plan choice and premium credit.   Also, when you participate in challenges to help you achieve and maintain your health and well-being goals, you can earn up to $125 in amazon gift cards.

If anyone would like to check if they already filled out their PHA since October 1 last year, here is how to check:
YP4H.osu.edu--> Log into My Plan-->log in with name.# and password-->click on Points History-->scroll till you see Complete Your Assessment PHA-Online-->it will show you the points received and date completed if it is done.
