Re:  Ohio State Faculty and Staff Still Have Time to Save $360!


Nov. 30 may be tomorrow, but it's not too late for faculty and staff to complete their 2011 Your Plan for Health (YP4H) Personal Health Assessment (PHA) to ensure they receive the full 2012 PHA Medical Plan Premium Credit of $360. Please use the information below as a guide to remind faculty and staff about the steps to take to complete the PHA.


*         Faculty and staff can receive their biometric health values in two different ways.

o   Schedule an on-campus YP4H Biometric Health Screening. Screening appointments are full for November, but there are still some available in December. To schedule a free on-campus biometric health screening appointment, visit YourPlanForHealth.com<http://www.yourplanforhealth.com> and click Biometric Health Campus Screenings on the right-hand side of the page.

o   Schedule an annual physical with their personal physician. Faculty and staff can receive their biometric health values from their personal physician in lieu of having an on-campus biometric health screening.

*         Direct faculty and staff to complete the PHA after receiving their biometric values. If faculty and staff are unable to complete the PHA by Nov. 30, encourage them to do so as soon as possible to start receiving the PHA Medical Plan Premium Credit of $30/monthly pay ($13.85/biweekly pay). Remember, an on-campus biometric health screening and completing the PHA are two separate activities. PHAs must be completed each year to continue receiving the PHA Medical Plan Premium Credit. To complete a PHA, log in to YourPlanForHealth.com<http://www.yourplanforhealth.com> and click Personal Health Assessment under Get Started.

We will be sending a reminder e-mail to all benefits-eligible faculty and staff who have not completed a 2011 PHA on Wednesday, November 30.
