[Intl_DxMedPhys] MQSA/EQUIP question

Hello Physics List Serv,
Question for all of you MQSA/EQUIP experts - recently our inspector asked us to add the QC and image review dates on our LIP QA/QC attestation form because they want to make sure there is continuity of review of QC and images. I showed where the date range was from our facility QC and image review. But they said it doesn't prove that the LIP reviewed it. They think the date range on this document is the ground truth that the LIP reviewed the date range as listed on the attestation. Do you include date ranges on your LIP attestation?

On a side note, does anybody list all your facilities and mqsa id's on one form and have your LIP sign this once a year? or do you have a separate attestation for each facility and have the LIP sign for each facility prior to inspection?
