[Intl_DxMedPhys] [External] GE MyoSPECT questions

I have seen this before on a GE D570 CZT camera. Each detector module is a 2x2 array of detectors. You have lost a detector.
I think it requires service support.
I believe in my case the service engineers reloaded corrections etc, rather than replacing a module. It has happened once in over 10 years of use.

I acquired a small Jaszczak phantom on a GE MyoSPECT camera and the reconstructed images exhibit severe streaking. One of the camera "projection" images seems to have a missing quadrant. I've attached that image. So my questions are:

What's going on here - what causes ¼ of the image to be zeroed out?

Any suggestions on how to acquire a good quality Jaszczak? My setup is really cobbled together. Is there a phantom acquisition mode I can access somehow or is there a jig available for this camera?
