[Intl_DxMedPhys] GE MyoSPECT questions

I acquired a small Jaszczak phantom on a GE MyoSPECT camera and the reconstructed images exhibit severe streaking.

One of the camera "projection" images seems to have a missing quadrant.  I've attached that image.

So my questions are:

has anyone seen this type of failure before?

What's going on here - what causes ¼ of the image to be zeroed out?

Any suggestions on how to acquire a good quality Jaszczak?  My setup is really cobbled together.  Is there a phantom acquisition mode I can access somehow or is there a jig available for this camera?
