Work in this area is ongoing... The forthcoming revision IEC standard for Radiography and Radioscopy equipment will almost certainly include recommendations that manufacturers include 3D dose data for standard modes of operation in accompanying documents. But - that won't affect current models.
There will also be a AAPM TG forthcoming soon to formalize the recommendations from 238. We expect that the recommendations (especially for C-arm systems) will be to:
Check constancy of Kar and KAP for spins under standard conditions
Measure dose free in air (under consistent conditions) at acceptance and annually
Consider measuring planar average dose in a phantom at acceptance (ideally under same conditions as dose free in air to allow calculation of phantom conversion factors).
I gave a talk at the '24 spring clinical meeting on adding these dose measurements and IQ measurements into clinical practice for evaluation of C-arms capable of 3D recons. Our team is seeking clarification on the required physics evaluations for C-arms with 3D Cone Beam Computed Tomography (CBCT) capabilities in South Carolina, as outlined under DHEC Regulation 61-64.
We represent a client that manufactures these devices, and they have been informed by the state that equipment evaluation reports must include 3D dose data. However, we are finding limited specific guidance in the regulations regarding CBCT-specific testing protocols, particularly for dosimetry requirements.
Could you share your insights or experiences with the following?
1. What specific tests are you performing for 3D C-arm CBCT systems during annual physics evaluations?
2. Are you required to provide 3D dose data in your evaluation reports, and if so, what methods or standards (e.g., AAPM TG 238, manufacturer protocols) are you using?
