[Intl_DxMedPhys] 131-I causing electronic failures?

As a follow up for everyone's info:

The question I had arises from a consent form I saw (below): We only use ~30mCi for our peds 131-I theranostics procedure. So I don't know where this wording came from. My guess is there is a single consent form for all 131-I procedures (including the therapy ablations), which there shouldn't be. Or they're recycling a therapy form.
