Hi Bob, 

I agree with Aaron about not second guessing the publisher. I looked at the subject headings and catalog record for the "ha-Yasmin ha-ḳadosh " and they appear to indicate that the author is Iranian in origin, and that the book is an autobiography which might account for the different vocalization of the surname. I am not that familiar with all the types of Iranian Jewish surnames. 

I personally would use the romanized form as found since we do not have any concrete evidence that the form is incorrect. Very often found romanizations within the resource being cataloged do not match AL/LC systematic romanization, but we are told that in the absence of a found romanized form in one of the reference sources listed in LC-PCC 9.2.2.5.3 we use the romanized form that is found in the resource. And the NAR can always be updated later. 

You could also contact the publisher to verify the romanized form and change the heading after you hopefully receive a response.

Folks: 

I encountered a problem this morning. I have an author whose name 
appears both in Hebrew and in Latin script. The problem is that the 
romanized form has an obvious error. In Hebrew, Yaakov Levi but in 
Latin script, Yakov Levu. I've looked on the web, and I haven't found 
any Latin script versions of his name. So what should the heading be? 

A workable solution would be to code the record as provisional and 
make the Hebrew script form of the name the provisional authorized 
access point, but there may be better ways. Any ideas?
