Dear all,
This is a reminder about the form for self-identifying as High-Risk (see below). Several points to keep in mind:
* This request is currently only directed at individual scheduled to teach next academic year. There will be future opportunities for those with other duties.
* If you believe you are high-risk, but would not seek accommodations, you do not need to submit the form. (Or could consider submitting later, if you change your mind.)
* The form should be sent to Peggy Link (link.39). You are not required to send it to anyone else in the department.
* While the request asks to submit by this Friday (6/19), there will be a chance to submit later. The College is attempting to get an estimate of the number of people. If you are sure you will self-identify, please try to submit the form on time. This will help with the estimate.
Finally, the Department Instructional Task Force is working on a plan for instruction next year. While the plan in not finalize and anything must be approved by the College, the plan is trending toward a large fraction of our instruction being online.

Dear All,
As we plan for activities later this year, we need to consider the safety of individuals in high-risk groups. This includes people who live with or care for people in high-risk groups. This will be important for planning our teaching and considering our other working duties and schedules. The College has asked us to perform an initial estimate of individuals involved in teaching that self-identify as part of a high-risk group. For the moment, we are only asking individuals (GTAs, Lecturers, faculty, staff) who are expected to be involved with teaching during the next academic year. We will request this from other individuals at a later date, but you should be thinking about it.
For the purpose of this exercise, we are using the guidelines from the CDC. Please see the attached form and you can find the CDC guidelines here<https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-at-higher-risk.html>. If you wish to self-identify as part of a high-risk group, please fill out the attached form and submit it to Peggy Link (Link.39) by Friday June 19th. Please note, this form needs to be submit so we know who is requesting accommodations. If you are not seeking special accommodations, you are not required to submit the form.
