Openings still available for Sept 12 - Physics Dept 1080 PRB biometric screening (tomorrow they open to all OSU employees)

Faculty and Staff on OSU Health Plans,
     There are still several biometric appointments available for this Friday's (Sept. 12) biometric screenings in 1080 PRB. Smith Seminar Room.  Please follow the instructions below to sign up.
The appointments have not been visible to outside the Physics department.  However, after today, the appointments will be open to all OSU employees so they may fill up fast.  Let me know if you have any questions.

Faculty and Staff on OSU Health Plans,

On Friday, Sept. 12th,  the OSU Health Plan's Health Promotion Specialist(s) will be in 1080 PRB  (Smith Seminar Room) from 8:00-2:00 to do onsite biometric health screenings.  This info is needed for completing the Personal Health Assessment (PHA) which will allow you to receive your health care premium credit reduction as well as make you eligible for cash-back incentives when you participate in YourPlanForHealth activities.

Registration is now open online for this date. Click on "Start Here".  Then click "Schedule a biometric screening".  Log in with name.#.  Select "Appointments and Classes" tab on top.  Highlight "Biometric Health Screening".  In the box that states "Appointment Code" please type in PRBBHS9122014. Then click "Find Appointments or Classes".   This will show only the appointments for your event in your building.  Continue to schedule as usual.  If you experience any difficulty, please contact Customer Service at 292-4700, but you must have your Appointment Code or they will not be able to assist you.

Note that fasting is not required for these screenings.
