Biometric Health Screenings- JULY 9 in Physics Dept- 1080PRB

Physics Faculty and Staff,

On Tuesday, July 9th  the OSU Health Plan's Health Promotion Specialist(s) will be in our building (Smith Conference Room 1080 PRB) from 8:30 a.m. to 4:00 p.m. to do onsite biometric health screenings.  Note, if you have had a physical  within the past year, you can use those values to complete your Personal Health Assessment (PHA) at www.yp4h.osu.edu<http://www.yp4h.osu.edu>.  Completing your PHA will allow you to receive your health care premium credit reduction as well as make you eligible for cash-back incentives when you participate in YourPlanForHealth activities.

Registration is now open online for this date.  In order to register for your screening, go to www.yp4h.osu.edu<http://www.yp4h.osu.edu>.  Scroll to bottom of page.  On left hand side of the footer under "YP4H Resources,  click "Biometric Health Screening".   After logging in, search for available screenings on July 9th under  Physics Research Building.   Please register promptly to ensure that employees of our department have first chance at these appointments.   You will receive an e-mail reminder one week before and the day before your appointment.

Fasting is not required for these screenings. Let me know if you have any problems registering for an appointment on July 9 or have any questions.
