Some more Biometric appts. have just been added, for Sept. 13 in PRB 4138

Physics Faculty, Staff, Lecturers, and Postdocs on OSU Health Plans,

Today I learned that the biometric screenings appointments for Friday, Sept. 13 in 4138 PRB had been filled over the weekend.
However, OSU Health Plan has just now arranged for a second nurse to come over from 11:00-4:30.  So there are now more biometric appointments in the system, ready to be reserved.

To Physics Faculty, Staff, Lecturers, and Postdocs on OSU Health Plans,

On September 13th, a OSU Health Plan's Health Promotion Specialist  will be in the Physics department for a second time this year to do onsite biometric health screenings. So if you missed the one last month you will have another convenient location to get your values for your PHA.  This screening will be in room 4138 PRB from 8:30 a.m. to 4:00 p.m..  Remember, if you have had a physical  from your PCP within the past 12 months or a biometric screening within the past 12 months, you can use those values to complete your Personal Health Assessment (PHA) at www.yp4h.osu.edu<https://urldefense.proofpoint.com/v1/url?u=http://www.yp4h.osu.edu&k=ux7ohqYFcw1oDo0gOpSLlw%3D%3D%0A&r=W1G%2Fv9EXDqism1vD24%2BS8CXlGFOkYzuixzUuep%2B1ZpE%3D%0A&m=gZDf%2By9m0QSw9EzSDDzaVhBZNz2njEEwc42uAnctRNw%3D%0A&s=9c2c770f544da146dd903cbb638cbd805fd6a376311275a8d7dd3202d1b39ece>.  Completing your PHA will allow you to receive your health care premium credit reduction as well as make you eligible for cash-back incentives when you participate in YourPlanForHealth activities.

Registration will be open online after ~4:30PM today.   Please register soon as the system is open to all of campus and spaces are filling up fast.

On left hand side of the footer under "YP4H Resources,  click "Biometric Health Screening".
After logging in, search for available screenings on September 13th under  Physics Research Bldg.

 We want Physics employees to have first chance at these appointments, so please register promptly.
