Open Enrollment ends at 11:59 p.m. this Friday, December 2. Please remind faculty and staff to make their 2012 benefit elections before then to ensure they have the coverage they want for 2012. Benefit elections can be made online at hr.osu.edu/oe<http://hr.osu.edu/oe/> or via paper enrollment forms that can be downloaded from the same web site.

If faculty and staff choose not to make any changes to their benefits for 2012, the only action required is for them to make an online designation of their life insurance beneficiary(ies), as the university is changing life insurance vendors effective January 1. This is their opportunity to electronically update their life insurance beneficiary(ies). For those who wish to continue participation in Flexible Spending Accounts, re-enrollment is required for 2012.

To designate a life insurance beneficiary(ies), log in to eBenefits at hr.osu.edu/oe<http://hr.osu.edu/oe/> and click Edit beside Group Term Life. You may designate individuals as Primary Contingent beneficiaries by allocating a percent value beside each beneficiary(ies') name you would like to designate. A zero value will result in no beneficiary election.

Once faculty and staff have made changes to their benefits and/or designated their life insurance beneficiary(ies), remind them to click Authorize and Submit to complete their changes. To Authorize and Submit benefits changes for 2012, click Continue on the Enrollment Summary page, read the Authorize Elections statement, and click Authorize and Submit at the bottom of the page.

Faculty and staff can review the coverage they are set up to have in 2012 through eBenefits. Once they log in from the eBenefits Home Page, there is a link to "View Confirmation Statement;" click on this link to view and print the information that was submitted to the Benefits department during the Open Enrollment period, or what will automatically default from their 2012 elections if no changes were made. The ability to view and print this benefit confirmation page will be available until December 31.

Faculty and staff will be receiving e-mails this week to remind them to designate a life insurance beneficiary(ies), Authorize and Confirm any benefits changes that were made, and remind them Open Enrollment is closing December 2.
