[Pedfac] OSU Privileges Form

Everyone,

The College of Dentistry has decided that credentialing of all full time clinical faculty is important for maintaining quality care and patient safety. All division chairs are part of the Credentials Committee and, as such, I was asked to submit a list of privileges for pediatric dentistry. NCH's form was used as the basis for the OSU list of privileges, along with CODA standards. Any privilege that does not match exactly with NCH, I have highlighted in yellow for your review.

This is only a draft and your feedback is welcomed. Please respond by the Friday with your comments - I will compile and recirculate any changes that are recommended.
