The answer depends on whether or not a preferred access point for a work is just the preferred form of a title or if it also includes the preferred form of a person or corporate body who is responsible for the work. 

If there are two works that have the same title but different people or bodies responsible for the work (MARC tags 100, 110, 111/240) then there is no conflict and nothing more needs to be done. 

But, 
a. if the authorized access point for a work does not include a responsible person or body , and there is another work entered under the same title then the conflict must be broken. RDA 6.27.1.9 and its accompanying LC-PCC PSS provide guidelines on how to provide additions to access points when needed to distinguish the access point for a work. ( 

b. tis also applies when an author creates more than one work with the same title proper and it is not considered to be part of a multi-volume set but rather different works. An addition will need to be added to the preferred form of the title of at least one of the works to differentiate it from another. This situation is a very common practice in rabbinic cataloging.

In Hebraica cataloging for many sefarim, works of rabbinic literature, that begin with the word "Sefer", we drop the sefer in the title when creating a uniform title (240 or 130). It is standard to do this. But what happens when there are two or more books with the same title, same subject headings, but different sub-titles, authors, publication information, and content? They have the same 245 $a and are on the same subject, but are entirely different works. Yet they get the same uniform title! Is it OK? Should it be done? Or should parenthetical qualifiers be put in ; if so, which?
