[OOD-users] Jupyter App -- satisfy password request && database connection error

Hi, everyone --

So I've followed the instructions for implementing the Jupyter app,
specifically configuring according to the github doc (i.e.

https://osc.github.io/ood-documentation/master/app-development/tutorials-interactive-apps/add-jupyter/customize-attributes.html
)

After 'connect to jupyter', I have mixed success, being prompted for a
password that I don't understand how to satisfy. Help appreciated.

To avoid squinting, the URL is replicated below:

Thanks
