I adjusted the setting so that those representing multiple districts can make access multiple times.

In order to complete surveys for the multiple districts I serve, I had to “clear 
the cache” at the completion of each survey.  

Be forewarned >>> You will lose ALL historical data.  Here are the steps I used:
Upper toolbar:
•  Click on “Tools”
•  Scroll down & click on “Internet Options”
•  Look for “Browsing History – Delete temporary files, history, cookies, saved 
passwords, and web form information.”  Click on “Delete…” [I check ALL boxes…]
     o  Preserve Favorite website data
     o  Temporary Internet Files
     o  Cookies
     o  History
     o  Form Data
     o  Passwords
     o  InPrivate Filtering Data
•  Click “Delete” and wait…
•  Click “OK”

FYI:  It said if you are a coordinator for more than one district, you can 
complete the survey for each district.  It would not let me, so I tried using a 
different browser and it worked.
